Prof Marso: If you take the current guidelines， the ADA currently recommends a HbA1c of less than 7% in people with type 2 diabetes. In fact， if you can get an individual down to 6%， it is beneficial. First way I would answer that question is to try and manage chronic glycemia down to about 7% HbA1c. In-hospital management is a lot more problematic. It is not clear to me that we should employ aggressive strategies to lower glucose levels in the hospital. There are a variety of epidemiological studies that suggest it may be helpful but there has been no intervention trial to educate me or enable me to do that as a provider safely and effectively and to what benefit.
