编者按:Charles Richard Conti教授曾为美国心脏病学学会(ACC)主席,多次参与长城国际心脏病学会议(GWICC)并发表重要讲题。在本届长城会上,《国际循环》记者就其相关讲题对Conti教授进行了采访。
International Circulation: What are the important medical humanistic values of patient ownership? What important roles do you think maintaining patient ownership play in the management of cardiovascular disease?
《国际循环》:患者所有权有何重要的医学人文意义?尊重和维护患者所有权在心血管疾病管理中发挥了怎样的作用?
Prof. Conti: What I mean by patient ownership is that I want the trainee or medical resident to feel that they have responsibility for the continuing management of any patient that is assigned to them. That includes even when they are off for the day, if something goes wrong with their patient, they know about it. They continue to manage the patient through some other individual. That is what I am interested in. I am interested in having people, rather than do shift work like you would in an emergency department and go home at the end of the shift and forget about everything, to go home and think about your patient and study the literature about your patient and keep in touch with the patient. That is important. I believe that makes for much better patient management as well as the education of the resident.
Conti教授:我强调患者所有权是想让实习医生或住院医生能够意识到自己有责任对分配给自己的任何一个患者进行持续管理的责任。即使他们结束了一天的工作,如果其所负责的患者出现了问题,他们也需要知道患者的情况。他们需要通过其他人来对患者进行持续管理。这是我所感兴趣的内容,我不想他们像在急诊室轮班工作那样下班结束回家后就忘了有关患者的一切,而是希望他们能够在回家后也能够想到自己的患者,学习与之病情有关的文献,并与患者保持接触。这一点是非常重要的。我相信,这将有助于实现更好的患者管理及住院医生教育。
International Circulation: How do you think clinicians should reconcile and maintain patient ownership with limited work hours effectively?
《国际循环》:在有限的工作时间内,临床医生应如何确保和及时有效协调和维护患者所有权问题?
Prof. Conti: The issue we have in the United States is that those of us who take care of patients have nothing to say about how often residents’ work schedules and how long they are away from work. I have no problem with them being away for a day or two, but I do have a problem when they are off in the middle of the week. If they are off on the weekend, very little gets done on the weekend in the United States, but a lot gets done during the week. By all means, give them their time off and have someone else take care of the patients on the weekend, but not during the middle of the week.
Conti教授:我们在美国所遇到的问题是,作为照顾患者的医生我们无权干预住院医生的工作时间表,对其应多久停止工作也无发言权。患者的负责医生离开一两天的话问题不大,但如果离开半周就会出问题。如果他们是周末不在的话,鉴于美国周末开展的工作很少、大部分工作都是在非周末时间完成,故可能影响不大。就周末而言,我们可通过各种手段给患者的负责医生休息时间,并让其他医生在周末照顾其患者。但是,如果负责医生要周中离开的话,则很难做到这一点。
International Circulation: You are an old friend of the Great Wall meeting. Can you tell us your impressions of this meeting?
《国际循环》:您是长城会的老朋友了,请您谈一下您对本届大会的印象。
Prof. Conti: I have been coming to the Great Wall International Congress of Cardiology every year since 2003. Professor Hu Dayi invites me to come and give a talk. In 2003, he and I and several individuals from the American College of Cardiology decided that maybe it would be nice to have a joint meeting of the ACC and the Great Wall. That transpired in 2004 and then every year since. I think it has been a great thing, not only for the Chinese, but also for the Americans to come and see how medicine is taught and delivered in China. And I can tell you, it is first class.
Conti教授:从2003年开始,我每年都来参加长城国际心脏病学会议,胡大一教授邀请我来进行专题讲座。2003年,胡教授、我以及其他ACC成员决定设立ACC@GW-ICC联合论坛。2004 <mailto:我以及其他ACC成员决定设立ACC@GW-ICC联合论坛。2004>年,首届ACC@GW-ICC联合论坛举办,随后每年举办一次。我认为,联合论坛的举办不仅对中国同道有重要意义,也有助于美国心脏病专业人员来看看中国是如何进行医学教育的。我认为,长城会是一个国际一流的大会。
